| An Elizabeth Township Area EMS subscription offers protection to you and your loved ones from the worry of large out of pocket expenses associated with medical transportation by ground ambulance. Our low annual fee of $45.00 (Family Plan) buys you, your spouse, dependents living in your household, and visitors freedom from deductibles and coinsurance. Medicare subscribers enjoy guaranteed assigned billing and no out of pocket expense for covered charges and for medically necessary non-covered charges (Physician certification required).
In addition to the aforementioned Family Plan, we also offer an individual plan for just $25.00, or a couple plan for just $35.00.
The Elizabeth Township Area EMS subscription plan does not cover:
- Transports lacking medical necessity or for patient convenience.
- Transports that by law or statute are not the direct financial responsibility of the patient. Medicaid clients are entitled to receive Medicaid-Covered ambulance services without paying a subscription fee or making a voluntary contribution.
- Transports by wheelchair, or stretcher van.
- Transports for which there is no primary third party insurer. Subscription plans are not insurance. They are a convenient method or prepaying coinsurance, deductibles, and certain non-covered Medicare services, which are typically several times more than the cost of an annual subscription. The patient remains responsible for the bill. Transports of subscribers for which there is no primary third party insurer will receive a 40% discount on billed charges.
This subscription is non-refundable and non-transferable.
